How to Qualify in Michigan

The steps in applying for medical cannabis in the State of Michigan are as follows:

Step 1: Complete a Physician Certification Form

  • Visit a physician with which you have a bona fide doctor/patient relationship
  • Receive certification that you suffer from one of the conditions that qualifies you to receive a patient registry identification card.

Step 2: Complete an Application Form

Include Proof of Michigan residency (one of the following):

  • A copy of your valid, lawfully obtained Michigan driver license
  • A copy of your valid, lawfully obtained Michigan personal identification card
  • A copy of your valid Michigan voter registration (if you submit a voter registration as proof of Michigan residency, you must submit proof of your identity. Proof of identity can be any government-issued document that includes your name and date of birth)

If you are designating a caregiver, include proof of the caregiver’s identity (one of the following):

  • A copy of the caregiver’s valid state-issued driver license
  • A copy of the caregiver’s valid state-issued personal identification card

Step 3: Submit the Required Application Fee

Include a check or money order made payable to: State of Michigan – MMMP

  • Patient Fee: $60.00
  • Caregiver Fee: An Additional $25.00
